The Relocation Service Coordination (RSC) Program is funded through Minnesota Medical Assistance (Medicaid), with little to no costs to our clients. When people are living in an institution such as a nursing home, but are ready to move back into the community, Global Health Link helps. We provide experienced case management that guides and supports people as they plan to move out of certain institutions and back into homes. To be eligible Medical Assistance beneficiaries should be moving out of a nursing facility, regional treatment center, hospital or ICF/DD (intermediate care facility for persons with developmental disabilities).
