Home and Community-Based Services in the Twin Cities Region
Global Health Link recognizes the importance of helping people live where they choose with appropriate services that assure their health and safety. People with disabilities have the right to receive support services in the most integrated setting possible. This is why we partner with the Minnesota Department of Human Services, Medicaid, Medical Assistance, and others to participate in plans and waiver services that support people living in their own homes in our community. Home and Community-Based Services
HCBS
Global Health Link is a licensed 245D Provider under the Minnesota Department of Human Services (DHS). This enables us to partner with certain Minnesota Medicaid Waiver Programs that fund a variety of important services for our neighbors. The Home and Community-Based Services waiver program covers a wide range of solutions, delivered at little or no cost to qualified clients in the Twin Cities region. The exact services available vary according to specific programs and individualized client service plans. Services may include:

24-Hour Emergency Assistance Waiver
Twin Cities 24-Hour Emergency Assistance Medicaid Waiver Service
The Minnesota waiver service for 24-hour emergency assistance provides on-call counseling and problem solving and/or immediate response for assistance at a person's home due to a health or personal emergency.

Minneapolis ICLS Waiver Services
ICLS sends workers to the homes of retirees to provide assistance and support in reminders, cues, intermittent/moderate supervision, and/or physical assistance to remain in their own homes.

Help with cleaning, meal planning/preparation and shopping for household and personal needs.

Provide medication assistance (e.g., medication refills, reminders, administration, preparation).

Help with budgeting and money management. Help with communications (e.g., sorting mail, accessing email, placing phone calls, making appointments).

Provide transportation when it is integral to ICLS household management goals and when community resources and/or informal supports are not available.

Continual supervision and physical assistance with bathing, as needed.

Provide reminders or cuing systems to help clients complete activities of daily living without always doing it for them.

Help our client adopt ways to meet their needs. Encourage self-sufficiency and reduce reliance on human assistance.

Intermittent physical assistance with dressing, grooming, eating, toileting, mobility, transferring and positioning.

Monitor the person’s health according to written instructions from a licensed health professional and report any significant changes as instructed.

Otherwise, identify changes in the person’s health needs and notify the case manager and/or informal caregivers as needed.

Coordinate or implement changes to mitigate environmental risks in the home.

Help the person access activities, services and resources that facilitate meaningful community integration and participation.

Help the person develop and/or maintain their informal support system.

Night Supervision
Minneapolis Overnight Caregivers
Overnight assistance and supervision provided by an awake staff in a person’s own home.

Assistance with activities of daily living, including eating, toileting, grooming, dressing, bathing, mobility, positioning, and transferring

Assistance with instrumental activities of daily living (IADLs), such as meal planning, preparation, and cooking; shopping for food, clothing, or other essential items; laundry; housecleaning; assistance with medications; etc.

Carrying out the person’s positive support programming and transition plans

Reinforcing skill development supports (e.g., individualized home supports)

Positive Support Services
Behavioral Support / Positive Support Services
We develop, implement, and monitoring person-centered, individually designed, proactive plans to address challenging behaviors. A Global Health Link positive-support professional develops this plan to help clients enhance their quality of life through processes of teaching and increasing positive behavior. When possible, our clients take the lead in the process. Our positive-support professionals visit our clients at their homes (or in the community) in greater Minneapolis to deliver positive support services / behavioral support.

Use of person-centered approaches that incorporate a comprehensive, functional behavior assessment of both positive and challenging behavior

Increase in the person’s quality of life

Decrease of the challenging behavior

Development of a Positive Support Transition Plan, when indicated, to phase out the use of restrictive interventions approved for use on a temporary basis

Respite Waiver Services
Respite Services for the Caregivers of Retirees in Greater Minneapolis
This refers to short-term care services provided to a person when the person’s primary caregiver is absent or needs relief.
